Please read the following instructions carefully and follow-up with the
appropriate training. You wouldn't want the elephant to get involved...

SLAC requires that employees and users receive
appropriate safety training and complete a Job Hazard Analysis and
Mitigation (JHAM) form
-
Anticipating this revised requirement, a Safety Training
Task Force was formed in November 2004 to examine implementation
requirements for BABAR; their report was received on January 6, and
discusses job categories, corresponding training requirements, supervisors,
and documentation requirements, along with accountability and record keeping

The basic requirements for all BABARians are:

Complete reading and related activities, most of which can
be done before arriving at SLAC
-
Take the EOESH, GERT, and ES&H Electrical Safety Training for
Non-Electrical Workers (on
demand computer-based versions of these courses are available at SLAC only, contact Donna Hernandez)

Review the appropriate template JHAMs and have the document
signed by one of the people named on the checklist
-
There are template JHAMs available to cover most
circumstances encountered by BABAR users (routine office, routine shifter,
and routine system specific), which should make this process straightforward
-
We urge
you, in the strongest terms, to complete these steps at the beginning of
your next visit to SLAC if you have not done so already
-
Complete individual routine Office Job Hazard Analysis and Mitigation
(JHAM) for all on site BABARians
-
Complete EOESH, GERT, and ES&H Course 239, Electrical Safety Training for
Non-Electrical Workers for all on site BABARians
-
Complete Commitment to Safety forms for all on site
BABARians

Complete requirements for routine Shifter JHAM and required
training for all BABAR shifters
-
Complete requirements for System JHAM and required training
for all BABAR system workers

The BABAR Spokesperson is designated by SLAC as the on-site
supervisor for BABAR users. This responsibility is delagated to a line
manager in the BABAR organization for various types of on-site activities as
specified in the table below.

Review the table below to ascertain the level of training
required for BABAR users, the designated immediate supervisor for training, and the
documentation requirements. Note that you may require multiple supervisors,
if you are involved in different kinds of work on BABAR. For example, if you
are working on a detector hardware system and take shifts, you will fall
under categories (b), (c), and (d), with corresponding supervisors (BABAR
management, run coordinator, and system manager respectively) and
documentation requirements.

Definitions for categories:
-
Short-term visitor: Attends no more than one collaboration
meeting per year or equivalent
-
Office worker: Only works in the ROB, Bldg 280, or one of
the other office areas and not in IR-2 or other laboratory spaces
-
Shift-taker: Stands shift on the BABAR detector in IR-2
-
System worker: Does maintenance work on a detector system in
IR-2 or in a system laboratory space
-
R&D worker: Does development or testing work in a lab
setting or temporary system space

of ES&H 239 are also possible; please contact the Run Coordinators to make arrangements.

Customize template office JHAM [doc,
pdf] as you deem
appropriate. You should retain the reference to the shifter JHAM if you plan
to run shifts on BABAR. You should retain the system specific JHAM if you
intend to do work on the detector, substituting the appropriate three-letter
acronym in the description. SLAC/BABAR members may opt to prepare an
individual JHAM for their normal working environment in connection with
their SLAC Research Group.

Bring your checklist
and the JHAM for signature to a BABAR management member or SLAC Group
Leader (for SLAC employees), and you will
be eligible for normal office
activities described in your routine office JHAM
-
Bring signed documentation to Donna Hernandez or SLAC Group
Leader (for SLAC employees). SLAC Research Groups will provide a summary of
signed documentation to Donna Hernandez.

Review BABAR shifter JHAM [doc,
pdf], IR-2 AHA [doc,
pdf], and complete any required
reading and training either before your next BABAR shift or, in the future,
as part of your normal shift training. The Run Coordinators will be you
supervisor for this activity and will define the shifter training
requirements.

Bring signed documentation to Donna Hernandez or SLAC Group
Leader (for SLAC employees)
-
You will not be allowed to take shifts without completing
these requirements, including current GERT training.
-
If you intend to work on a detector hardware system or
electronics system, review the shifter JHAM [doc,
pdf], the IR-2 AHA [doc,
pdf], and any relevant System JHAM and complete any
required reading and training. The relevant System Manager will be your
supervisor for this activity and will define the job specific training
requirements.

Bring signed documentation to Donna Hernandez or SLAC Group
Leader (for SLAC employees).
-
You will not be allowed to work on your system without
completing these requirements, including current GERT training.
-
If you intend to work on a development project or
electronics system at a SLAC-site other than IR-2, develop or review the
relevant job-specific JHAM and AHA, and complete any
required reading and training. The relevant Development Manager (could be a
System Manager in some cases) will be your supervisor for this activity and
will define the job specific training requirements.

Bring signed documentation to Donna Hernandez or SLAC Group
Leader (for SLAC employees).
-
You will not be allowed to conduct your development work
without completing these requirements, including current GERT training.

The SLAC Training Assessment Database (STA) has been
augmented with the additional capability to provide records of BABAR
training status and JHAMs for BABAR members, associates, and honorary
members. Requirements are automatically assigned based on the work category
given to the BABAR collaborator. This tool will allow BABAR management, Run
Coordinators, System Managers, and institutional PIs to verify the training
records and therefore suitability to perform work.

If you have comments and questions concerning the database,
or need to request changes to a training record, please contact
Donna Hernandez. Note that we are
developing report views of database information that will be helpful to Run
Coordinators and/or System Managers. Also, the left-hand sidebar of the
database web page lists the people for whom you are the responsible
supervisor. If a name is shaded, this means that there is a problem with the
training record, e.g. an expired training or JHAM.
